Use Google Forms with Google Sheets Reviews

I was able to make Q2 as Required, however the lab did not recognize the setting change. I tried multiple times but I still couldn't score 100. I tried submitting the survey without a last name and the survey denied submission. It was clear that I did it correctly, but still the lab did not approve it.
