Absolute shocker! This has not been updated in a long time and the required AWS Community AMI no longer exists. In the end after several false tries I used bitnami-lampstack-5.4.18-0-linux-ubuntu-12.04.2-i386-ebs-2-ami-97d14aad-3-ami-c5375bff and this seemed to suit the purpose OK. Also the IP address given to connect to the SQL server at the end may be incorrect (1 in 250 chances of it being correct). Really really needs to be updated.
