Be sure to replace the default network with my-internal-app and subnet-a, when configuring the utility-vm, otherwise, you won't be able to ping and curl everything in subnet-b, as it will output on the default network interface, and never find anything in subnet-b. Instructions only state to add a network interface, when creating the utility-vm, but you have to replace the default config to be able to reach the vm and lb frontend in the other subnet.
