I had drama opening a second shell to test the Cloud SQL proxy. It could have been my local machine. The second cloud shell was painfully slow and the cloud page was unresponsive. I was suspicious that terraform and the proxy were too resource intensive for the shell instance.

the order of the steps are not correct, you need to be in the terraform folder 1st to get the db instance name to setup the connection
